I do hereby waive and release Kemberly, Terry and Amanda Brown, pet care givers, (hereby known as THE BROWN TEAM)
from and all liabilities of any nature for the actions of myself, my pet(s), or someone who holds a key to my home, except
those arising from negligence or willful misconduct on the part of THE BROWN TEAM.
THE BROWN TEAM agrees to provide all services in a kind, reliable and a trustworthy manner.
THE BROWN TEAM also agrees to attempt to contact client or other party stated upon any concerns regarding their home and/or pet(s) before action outside of normal duties is taken by THE BROWN TEAM. In the case of inclement weather or a natural disaster, I authorize THE BROWN TEAM to use reasonable judgment for the care and the well being of my pet(s) and/or house – which is choice #1, unless otherwise stated here as choice #2 or #3 – see company polices.

walk them outside of your property without tags attached, correct collar and information needed in yellow folder.
I would prefer THE BROWN TEAM to transport my pet (s) to my veterinarian (info below) for the medical assistance in the
case of an emergency or/and care. I give permission for THE BROWN TEAM to use a vet of their choice if my vet is
unavailable. I also give my permission to THE BROWN TEAM to approve any veterinarian treatment at my absence, if I am

Please leave out on the counter this form, leashes (please have proper collars on your pets with location tag information), food, snacks, medicine, small trash bags, cleaning supplies - along with a broom, dust pan and vacuum that is in plain sight, etc... Some of the clean up happens on THE BROWN TEAM’S last visit (like sweeping), so please advise if coming home early.
PLEASE MAKE SURE ALL DOORS AND WINDOWS ARE LOCKED UPON LEAVING. Consider turning water off at main or where you can - hot water tanks, under sinks, washers and toilets. Appliances that are not being used - turned off. Close doors where possible that your pet really does not need to go in – less rooms for THE BROWN TEAM to check on at each visit. Make sure doors cannot close where pet (s) come and go (cat’s and their litter boxes).
A log of visits and duties preformed, will be left for
your viewing upon returning home.

Prices will vary for daily stays in Kemberly’s home (not a kennel) if the need arises. Pet(s) must be picked up by 10 a.m. so as to not incur another day charge.
Shampoo $20 small pet, $25 large pet. Groomer/vet or errand runs, $20.
Holidays will be an extra $10 per visit: Easter Day, Mother’s and Father’s Day, Memorial Day, 4th of July day. Labor Day, evening before Thanksgiving and day of Thanksgiving, evening before Christmas and day of Christmas, New Year’s eve and the day of the New Year.
Payment for services is due in full upon THE BROWN TEAAM’S first pet visit to client’s home, post dated check (the day of your return) may be left on the counter. There will be no refunds due to early returns.
